Bij switch naar PO: clindamycine SD PO

Met prothesemateriaal:

flucloxacilline HD IV (+ rifampicine SD PO/IV zodra wonde droog)
Bij switch naar PO: ciprofloxacine HD PO + rifampicine SD PO/IV

Igv milde type 4 allergie (bv. milde rash na enkele dagen):
cefazoline HD IV

Igv type 1 allergie (bv. anafylaxie, urticaria,...):
cefazoline HD IV

Igv ernstige type 4 allergie (bv. Stevens-Johnson, TEN, DRESS,...):
vancomycine IV
