CAP 4: zeer ernstige pneumonie-hospitalisatie op ICU noodzakelijk

Eerste keuze

amoxicilline-clavulaanzuur SD IV (5-7 dagen) + clarithromycine HD IV (5 dagen)

Voorafgaandelijke antibiotica of hospitalisatie:
- ceftriaxone SD IV (5-7 dagen) + clarithromycine HD IV (5 dagen)

Patiënten met structureel longlijden (bv. bronchiectasiën):
- piperacilline-tazobactam HD IV (5-7 dagen) + clarithromycine HD IV (5 dagen)

Penicilline allergie

Igv milde type 4 allergie (bv. milde rash na enkele dagen):
cefuroxime axetil HD IV (5-7 dagen) + clarithromycine HD IV (5 dagen)

Igv type 1 allergie (bv. anafylaxie, urticaria,...):
cefuroxime axetil HD IV met eerst testdosis (5-7 dagen) + clarithromycine HD IV (5 dagen)

Igv ernstige type 4 allergie (bv. Stevens-Johnson, TEN, DRESS,...):
moxifloxacine SD PO/IV
