Septische arthritis en acute osteomyelitis kind >3 maand en ≤4 jaar

  • therapeutisch en diagnostisch aspiraat  en/of biopt is essentieel
  • steeds AB in hoge dosis!
    • cefadroxil = 150 mg/kg/d in 3 dosissen (max 4 g/d)
    • flucloxacilline = 100 -150 mg/kg/d in 4 dosissen (max 12 g/d en 2 g/dosis)
    • PO flucloxacilline enkel na orale absorptietest en indien hoge dosis gastro-intestinaal verdragen wordt

cefazoline IV

Opmerkingen

  • indien gunstige evolutie kan na minimum 4 dagen naar PO therapie worden overgegaan

  • PO behandeling met cefadroxil of flucloxacilline (bij S. aureus) of clindamycine (enkel bij MRSA en/of andere opties niet mogelijk)

  • zo flucloxacilline PO, overweeg vooraf orale absorptietest.

  • duur behandeling (IV+PO):
    • septische arthritis: 2-3 weken
    • osteomyelitis: 3-4 weken (stop indien CRP < 20 mg/L). Indien gecompliceerd verloop overleg met kinderinfectioloog voor duur behandeling.
