Septische arthritis en acute osteomyelitis kind >4 jaar

  • therapeutisch en diagnostisch aspiraat  en/of biopt is essentieel
  • steeds AB in hoge dosis!

flucloxacilline IV

Opmerkingen

  • indien gunstige evolutie (verbeterende kliniek en dalend CRP) kan na minimum 4 dagen naar PO therapie worden overgegaan.
  • bij infuusproblemen: cefazoline IV gezien minder caustisch
  • PO behandeling met cefadroxil of flucloxacilline (bij S. aureus) of clindamycine (enkel bij MRSA en/of andere opties niet mogelijk)
  • zo flucloxacilline PO, overweeg vooraf orale absorptietest.
  • duur behandeling (IV+PO):
    • septische arthritis: 2-3 weken
    • osteomyelitis: 3-4 weken (stop AB na minimaal 3-4 weken, indien CRP dan < 20 mg/L). Indien gecompliceerd verloop overleg met kinderinfectioloog voor duur behandeling.
